Patient Information

Surgical Patients

To ensure timely registration, we ask patients to bring the following documentation:

  • Insurance ID card(s)
  • Drivers license or personal ID card
  • Any paperwork from your physician, such as test results
  • List of your medications
  • Payment for your insurance co-pay

Please remember to follow any specific preparation and/or dietary restrictions communicated by your physician’s office at the time of scheduling. Failure to do so may result in the rescheduling of your appointment.

Lab and Imaging Patients

Lab and imaging patients are asked to arrive at least 15 minutes prior to your procedure time to complete registration paperwork. To ensure timely registration, please bring the following documentation:

  • Insurance ID card(s)
  • Drivers license or personal ID card
  • Physician order forms
  • Payment for your insurance co-pay, if any.

Walk-in Imaging Patients

The radiology department at Methodist Hospital for Surgery does offer walk-in imaging services. To ensure timely registration, please bring the following documentation:

  • Insurance ID card(s)
  • Drivers license or personal ID card
  • Physician order forms
  • Payment for your insurance co-pay, if any.

Please note that pre-certification may be required.

Special Needs Patients

We are happy to accommodate any special needs of a patient. We ensure accessibility and effective communication regardless of your situation. Please notify the hospital of your special needs prior to arriving on your day of surgery. Methodist Hospital for Surgery uses the following number for the deaf and hearing impaired: (469) 248-3797.

Outpatient Physical Therapy Patients

Outpatient physical therapy patients are asked to arrive at least 10 minutes prior to your appointment time to complete registration paperwork. To ensure timely registration, please bring the following documentation:

  • Insurance ID card(s)
  • Drivers license or personal ID card
  • Physician prescription
  • Payment for your insurance co-pay, if any

If you have a physician prescription for physical therapy you can call 469-248-3959 to schedule an appointment. For the initial appointment it is best to wear comfortable, loose fitting clothing.
